Student Forms & Transcripts
The following list reflect standard forms that are required by the College to document and perform activities associated with a student's college experience.
Electronic Forms
We are in the process of moving all forms to an electronic format. To complete a form electronically, you must FIRST DOWNLOAD THE FORM to your device. Otherwise your data won't save.
You may still sign printed forms by hand, but if you prefer to sign electronically, please select one of the following options:
- Type your name in the signature field and submit the form by sending it to registrar@cambridgecollege.edu from your Cambridge College email account.
- Download Adobe Acrobat Reader here (free) and use this guide to create your own electronic signature. The form can then be submitted to the Registrar's Office by whichever means is easiest for you.
Graduation
- Intent to Graduate Form for June 2024 degree conferral. Submit this form ONLY if you will complete ALL courses for this degree or certificate in May 2024, and will not be completing courses in the summer. This form is not to confirm your attendance at the commencement ceremony, but so that we know to audit your record and put a June graduation date on your record if you meet the requirements. Only degree-seeking students are eligible to participate in the ceremony.
- Petition to “March Only” in June Commencement Form. Submit this form if you will complete your degree in August 2024, but would like to participate in the commencement ceremony in June 2024. Please do not submit the form unless you meet the requirements explained on the form, including being registered for the summer. Summer registration opens April 1. Only degree-seeking students are eligible to participate in the ceremony. This form includes an intent to graduate form for August, because that is when you will actually be completing your degree.
- Intent to Graduate form for August 31, 2024 degree conferral. Submit this form ONLY if you will complete ALL courses for this degree or certificate in August 2024, and will not be completing courses in the fall. This form will let us know to audit your record and put an August graduation date on your record if you meet the requirements. If you have already completed an August intent to graduate form as part of your “March Only” application you don’t need to fill out another one if you were approved to march.

Program/Course
- Add/Drop form -- paper form required after add/drop deadline (before the deadline, matriculated students must go to MyCC web portal).
- Accelerated Bachelor's to Master's Worksheet/Application - for undergraduate students applying to an accelerated master's degree program.
- Family Educational Rights & Privacy Act (FERPA) notice
- FERPA-Inspect -- Request to inspect and review education records
- FERPA-Parents -- Permission form to disclose records to parents
- FERPA-Release -- Permission form to release education records
- Non-Credit Training & Development Registration form
- Non-Matriculated Student Registration form -- for students not in a degree or certificate program.
- Prior Learning Assessment form -- undergraduate portfolios and standardized exams for credit.
- Program Change form -- continuing students, use to request a change of your academic program or concentration.
- Registration form -- for all students registering after the add/drop deadline for courses that have not started.
Transcripts
There are three ways to get transcripts, depending on where and when you took classes. Please follow the transcript request instructions carefully to get your transcripts as quickly as possible.
Official Cambridge College transcript ordering through the National Student Clearinghouse
Official transcripts cost $5.00, plus a $2.50 processing fee, and an additional $1.00 fee if it’s an electronic transcript. You may order more than one transcript per transaction if they’re hard copies going in the mail, but you may only order one electronic transcript per transaction. Order now
Please note: once your transcript request is received, processing may take up to 10 business days depending on volume.
Unofficial Transcripts for Cambridge College Students
The Registrar’s Office must receive a completed Transcript Request Form before we can process your transcript. Remember to download the form to your device before filling it out.
- Please note that you will need your Cambridge College ID number in order to submit a request. If you don’t know your Cambridge College ID number, you may call the Registrar’s Office at 617-873-0101.
- Once your transcript request is received, processing may take up to 10 business days, depending on volume.
- Unofficial transcripts are free of charge.
- Need your unofficial transcript immediately? You may download a copy of your unofficial transcript from MyCC. Login, go to the “Registration” tab, and find the Unofficial Transcript section in the center of the page. Choose your division, and generate the transcript. There’s a link to a PDF version on the bottom left side of your unofficial transcript.
Transcripts for New England Institute of Business (formerly New England College of Business) students through Parchment
If you are a current NEIB or former NECB student, you will order official transcripts directly through the Parchment site or through Cambridge College via the National Student Clearinghouse, depending on when you attended. Please contact neib-registrar@cambridgecollege.edu with any questions.
Who should order official transcripts from Parchment?
- NECB or NGS alumni who had a last date of attendance through February 2020
- Students who solely attended NECB and/or The National Graduate School through 3/14/20 and not thereafter
- Students currently enrolled at Cambridge College who need documentation of grades earned at courses taken at NECB. Credits transferred into Cambridge College were transfer credits, which do not include grades.
