Student Forms

  • Add/Drop form -- form required after add/drop deadline (before deadline, matriculated students must go to MyCC web portal).
  • Change of Address form (or other student information).
  • Change of Program form -- continuing students, use to request a change of your academic program or concentration.
  • Family Educational Rights & Privacy Act (FERPA) notice
  • Immunizations form and instructions -- Massachusetts only (Cambridge, Merrimack Valley, and Springfield centers)
  • Independent Study Authorization form
  • Intent to Graduate form -- for January, June and August 2013 degree conferrals.
  • Leave of Absence/Withdrawal form -- important for students who find it necessary to take a term off or withdraw from the College.
  • Official Letter Request form
  • Petition to March Only -- may be requested for annual Commencement in June.
  • Payment form.
  • Prior Learning Assessment form -- undergraduate portfolios and standardized exams for credit.
  • Registration form -- for non-degree students, and all students registering after the add/drop deadline for courses that have not started.
  • Replacement Student ID form.
  • Return from a Leave of Absence form.
  • Transcript request form.
