Replacing I-94
If you have lost your I-94 card, it is very important that you replace it as soon as possible. The procedures and forms are available on the USCIS website.
Please consult an ISO staff member if you have any questions.
OPT
To apply for OPT please bring the following documents to the ISO office. You may pick up an application form at the ISO or download it from USCIS website.
Change of Status/Reinstatement
If you are changing your non-immigrant visa status to or from F-1 or if you are out of status and are in need of reinstatement, please download the instructions and application on USCIS website or consult an ISO staff member.
Add/Drop
Please fill out the form and return it to the Registrar's Office if you plan to add or drop a course. Please do so before the Add/Drop Date specified on the academic calendar for each semester.
Leave of Absence
Please fill out this form and return it to the Registrar's Office if you are taking a semester off. Also, please give a copy of the form to the ISO and meet with an advisor prior to withdrawal.
Transcript Request
Please use this form when requesting official transcripts. You will need transcripts if you have to renew your visa. Submit the form to the Registrar's Office.
Application for Invitation Letter
If you wish to invite your parents, friends, or relatives to your commencement ceremony or for a visit, please fill out the form and send it to the ISO.
Address Change
You must notify the ISO and the Registrar's Office of your new address within 10 days. Please fill out the form and mail or fax it to the ISO and the Registrar's Office.
CPT Authorization
Please fill out the form with your academic advisor and return it to the ISO.
Program Extension
If you need to extend your program, please fill out the form and return it to the ISO prior to the program end date on your I-20.
