Financial Aid Forms and Verification Process

2008-2009

  • Appeal Form - Students who need additional financial aid to cover educational costs not allocated in original award.


  • The Financial Aid Institutional Application - Required of all students each awarding year.







  • Low Income/Asset Form - If your income from the FAFSA appears unreasonably low; the student will be requested to complete this document.



  • Student Non-Tax Filer Form - If a student has earnings that do not require them to file a tax return and the student is selected for verification this is the form a student would complete.



    The Verification Process:

    Some students are selected for a process called Verification. This means that the financial aid office must verify certain information using your federal tax returns and a verification worksheet. The Financial Aid Office will notify you if these documents are needed. If you are selected and fail to provide us with the documents requested, you will not be eligible for financial aid



  • Verification Worksheets - The Financial Aid office will inform you if you have been selected for a process called Verification. This means you will need to provide us with your Federal Tax Returns, Tax Schedules, Verification Worksheet and possibly other documentation. Dependant students (students between the ages of 18-24 who live with parents)


    will complete the Dependant Verification Worksheet. Graduate students will complete the Independent Verification Worksheet if selected for Verification.
